Health Requirements for 2025-2026 School Year
Dear Parent/Guardian,
In preparation for the upcoming school year we would like to make you aware of the following requirements for your child to begin school this upcoming school year. As your child enters 6th grade, he/she will be required to have the following vaccinations:
Tdap (Tetanus, diphtheria, acellular pertussis) - 1 dose given no earlier than 10 years old
Meningococcal - 1 dose given no earlier than 10 years old

We strongly recommend having a physical examination completed each year and submitted to the health office to ensure the health and safety of each student. For students with the following medical concerns please fill out the appropriate paperwork:
Allergy - Food Allergy & Anaphylaxis Emergency Care Plan
Asthma - Asthma Treatment Plan
Seizure - Seizure Action Plan
Diabetes - Diabetes Medical Management Plan
For any other medical concern not listed above please include a letter from the physician indicating any accommodations that must be made for your child.
Students receiving medication at school, either prescribed or over the counter will require a doctor’s order. The paperwork for medication administration can be found on the district website on the health services page.
**Medication should only be given during school hours if it is a requirement by the physician, all medicine that can be taken at home, should be taken at home.
